Who is LEVY Health’s fertility assessment for?

Is LEVY Health covered by Progyny?
Yes, testing through LEVY Health is covered under your Progyny benefit. Please note
you will be subject to financial responsibility according to your plan. Please note that
you will need to provide your Progyny member ID and the authorization number for
testing. Both can be found on your Progyny confirmation statement.
What happens if I have questions or concerns during the process?
Should you have any questions and/or concerns at any stage during your LEVY testing,
our LEVY Health care team is available by phone or text at 415-969-3373, or through
email at support@levy.health. We will make sure to respond to you within one business
day. If you have questions regarding your Progyny benefit and coverage, please contact
your designated Progyny Patient Care Advocate (PCA).
What can I expect from start to finish when using LEVY Health, and what are the benefits?
Initially, you will need to complete the online health assessment. Following that, you will be required to provide us with your Progyny member ID and authorization number in your LEVY Health profile. Subsequently, you will be prompted to provide details about your appointment date and clinic. Afterward, a customized lab requisition will be generated for you and sent directly to either Labcorp or Quest. Once this step is completed, you will receive an email notification informing you that you may proceed with scheduling an appointment to have your labs drawn. Upon receiving your lab results, we will promptly notify you via email. At this point, you can print out the results and bring them to your appointment. Now you are best prepared for your first appointment at your clinic and ready to kickstart your treatment!
What kind of information is collected in the health questionnaire, and how long does it take to complete it?
The health questionnaire assesses all kinds of data about your medical background and
your fertility journey. All questions are based on medical guidelines and will be handled
confidentially. To complete the Reproductive Health Assessment you will need about 20
minutes.
When is the right time to go to the lab for my blood work?
After completing your health questionnaire, you can choose to visit one of our lab providers, either Quest or Labcorp. A lab requisition will be sent to the lab of your choice. Subsequently, you can schedule an appointment for your tests at any time. Please be sure to schedule your lab work no less than two weeks before your clinic appointment to ensure your lab results are ready.
How long will it take for my pre-screening lab results to arrive?
Receiving your results can take up to 2 weeks, so it is important to complete your testing at least two before your first appointment. We will inform you as soon as your results are ready via email and/or text.
